OOo QA - Template for WIKI docs

as a lot of wiki docs not even contain an responsible autor
nor a date 
.. i created a template with the lowest requirements, based
on the 
decision of the former OOo qa reloaded docu team - please
fit your 
documents to this requirements and use the template in
future time for a 
better appearance of our documents.
