Various readers (of greatest interest, Omnikey 3121) have
status indicator LEDs.  Is there an API function to allow
an application to control these?  E.g. it would be nice 
for the application to signal the user that the card was
accepted or rejected. Maybe some secret APDU...?

I am not aware of a feature like that.
You should contact the reader manufacturer to know if it is
possible
to control the LEDs.
