Thanks to Dave Kelsey for the new XDebug / PDT document

I was finally able to get XDebug working with PDT thanks to
the document that Dave Kelsey released
on the 2nd of October. Thanks Dave! My only suggestion would
be to expand and clarify the "2.4.1
Web Launch" section a bit. Other than that, everything
is Cheddar.
