note 71476 added to function.mysqli-real-escape-string

I am wondering why nobody explain how to use this command?
If I will do a query which will insert in a field named
title : the string <?php echo "123";?>, than
it will be dangerous or not to display that with echo in a
page?

   If I try that to output after I get it with
$row["title"] sometimes is it shown, sometime is
not ! 

So anyone please illuminate a noob path in PHP please.
