Can anyone help me in making a ste automated? Tell me how or where to find it.

I would like to know how to make a form that would let a
customer input 
photos, information such as description, address, links and
so forth 
and it would put all on a webpage for an add on a site. Then
it would 
connetct that page to a cegory link on our site in our
database.
