Pedicures are "Relatively Safe" Except for
Diabetics: IN DPM 

Proper sanitation rests on more than sterilizing or
disinfecting 
instruments between uses. It also can rely on a technician
washing 
hands between clients and regularly cleaning the pedicure
tub. If an 
instrument or tub is not properly disinfected, a previous
client's 
problem can spread.

"I think that's people's biggest fear" with
pedicures, said DR. 
TRACY WARNER of Podiatry Associates of Indiana. "But
if you're in 
normal, good health, for the most part, pedicures are
relatively 
safe." 

Still, she added, "it's not unusual" for her to
see patients with 
toenail fungus infections they think they acquired in a nail
salon. 
These can require two to six months of treatment. So Warner
and 
other doctors recommend that diabetics see a professional
for callus 
removal and nail trimming, and trust only polishing to a
nail 
technician. "Having a pedicurist do it sometimes can
set you up for 
problems," Warner said. "Maybe they're trying
to treat an ingrown 
toenail when they shouldn't be."

New Rule Will Require Periodic Enrollment ReCertification

A final rule published in the April 21 Federal Register
requires 
that providers and suppliers periodically update and certify
the 
accuracy of their Medicare enrollment information to receive
billing 
privileges. It also implements provisions to ensure that
Medicare 
providers and suppliers are qualified to provide the
appropriate 
healthcare services. "These statutory provisions
include 
requirements meant to protect beneficiaries and the Medicare
Trust 
Funds by preventing unqualified, fraudulent, or excluded
providers 
and suppliers from providing items or services to Medicare 
beneficiaries or billing the Medicare program or its
beneficiaries," 
CMS says. The rule will take effect on June 20, 2006.

Query: Archery Shoe

I have a patient with no significant major biomechanical
deformities 
nor complaints, but he heard of a shoe that is made
specifically for 
those engaging in archery.  I assume this is some
specialized 
stability shoe, but since I know only what the layman knows
about 
archery, I am asking for input.  Does anybody have
experience with 
archery or have a patient with this request in the past?

Query: Carrier Denied Apligraf Application

Our Medicare carrier has determined to withhold payments to 
podiatrists for the application of Apligraf, asserting that
this is 
a service only to be reimbursed when performed by orthopedic

surgeons. This is obviously prejudicial and likely unlawful.
Where 
do we go from here?

benefit reason; letter from the carrier, site posted
bulletin, 
etc.)? 

I would bring this up immediately to your CAC rep, state 
association, and APMA. Medicare carriers cannot discriminate
payment 
among provider types. This needs to be addressed promptly.

Our esteemed editor, Barry Block, DPM JD, has asked me if I

the pages of Podiatry Management. After much thought, and

have really enjoyed professional writing, I have decided
that such a 
feature in Podiatry Management may be just the vehicle I
have been 
looking for. This publication has become first-rate and most

colleagues tell me they read it cover to cover. I know I
certainly 
do. Second, this publication is noted not only for its
sharing of 
knowledge, but for its delivery of controversy and dialogue
as well, 
which you may have noticed, compels me often to add my two
