Give the neutral item a language.

The whole point of "neutral" is that it should
show up in all languages.

With this type of behaviour I would either end up with double visible content (keeping the neutral version) or a situation where it is absolutely critical that the material has been translated into every language (having no neutral material, only different translations) 'cause if it hasn't been, then people of a certain language will not see the material at all and the page "The page you requested is not available in your language but it is available in these languages instead&quot; rendered completely unnecessary.

My desired behaviour is that the neutral version is only visible if there is no translation for the current language, but the neutral version is merely the list of available languages and a neutral title.
