Brian, Yo u are right, something funny is going on. I connected a spare ps2 mouse directly and gpm and X were working ok. I still think the problem lies with RHEL4 though. Knoppix and XP behave correctly, using the KVM, which suggests something wrong with the RHEL drivers/kernel.
I am short on space so I cannot really have another mouse on my desk. I'll try to get a ps2->usb adapter and see if I have more luck that way.

Try without the
KVM if you haven't already. Some KVMs and motherboards have funny interactions.
Are you using a PS/2 keyboard and PS/2 mouse? If not, read on. We have some HP workstations running RHEL 4 where the PS/2 keyboard will only work if the PS/2 mouse is plugged into the motherboard. My XW4300 is one such workstation. I have a PS/2 mouse dangling on the back of the workstation, but I use a USB mouse. We were told by HP it was a Linux 2.6 kernel issue, but I believe it's an ACPI implementation issue in HP's BIOS. Long story short, I wonder if the opposite might be true on your setup. If you're keyboard is USB, maybe your PS/2 mouse won't work.
