I've had two colonoscopies and both times my colon-rectal surgeon used a pediatric size tube to look inside my colon. It is smaller in diameter and is more flexible. My main problem was a very tight anal muscle. Even after my LIS, it remains tight, but not anything as bad as it was before.
Sorry I am not familiar with the type of fissure you are describing, so I don't know any advice about using a heating pad.
