PubMed uses a specific algorithm behind the simple search.
Type 'breast cancer', and then go to the details tab. You
will see that PubMed search for breast cancer as keyword,
but also for 'Breast Neoplasms' as MeSH (he made the
mapping), and add also the keywords 'Breast Neoplasms' for
seraching the in process citations (those without MeSH yet)

Like this ("breast neoplasms"[TIAB] NOT
Medline[SB]) OR "breast neoplasms"[MeSH Terms] OR
breast cancer[Text Word]

I guess EndNote just looks for what you typed, strictly into
the fields you selected, without this kind of algorithm

The MEDLINE export format does not include the url of the
pubmed citation, neither the xml...

You have then to choose...
