RE: Vancouver

"Postgraduate Medical J" style is an EndNote style
based on Vancouver
which uses numbers in square brackets for in-text references
and
italicizes the journal name in the reference list.  You
could give that
a try and see if it meets you needs, or else edit your
Vancouver style
in the style manager (under a different name) to make the
changes you
want.

The Vancouver style in my EndNote (X.0.2) uses square
brackets but not
italicized journal/book names. It seems there are
variations. The one

None of the variations that I found uses italicized
journal/book names.
