Jayson,  

I have been investigating ways to bring the PMC ID into
EndNote. Zotero
will grab the PMCID, but it puts it into a field labelled
"Extras".
Unfortunately the "Extras" field is not included
in a RIS or Refer
export from Zotero.

I have done exactly what you suggest - added a custom field
to Journal
article references type and then modified an output style
modeled on the
examples on the NIH FAQ.

Barbara,

I'm not sure this would work with your workflow, but since
you are
willing to place references at the end, have you considered
using the
Subject Bibliography feature of EndNote?  Enter the Chapter
name or
heading you want to use for the end bibliography in the
Label field of
each record.  Use the Label field as your only subject.  You
can format
entries in a variety of ways.
